More info needed. Did you no more than want to replace the wall mount jack with a regular phone jack? If so, you can buy an all in one plate at a lowes or home depot. When removing the old jack, note which wires were connected to which terminals and metathesize the setup to the new device.

There don't come to be any external screws. It's a kitchen wall phone that I want to update. I seem to remember these slide onto a wall plate or something????

2 raised circles bulge from the wall plate the phone slides down on to these so if you apply a little pressure and slide the phone up it should come off the wall.

Then the plate should have visible screws to separation it from wall.

Do they make a wall plate that contains 5 speakers, phone, and a cable outlet?

I've seen one with 5 spieler, and cable, but not one with phone also.

You can assign your own. Look for decora keystone plates and just get the right keystone inserts for what you want. You will need a dual face one side can hold four of the speaker's terminals (4 red and 4 black) and you can fit the rest on on the other(1 red and 1 black speaker station, 2 cat5 and 2 coax. Otherwise I think I saw a plate like that from OnQ but it may cost more.
